*IP addressing is an additional Service to LB DSL’s standard SDSL, iDSL, T-1, Fractional T-1, and ADSL SOHO Services. End User Circuits using IP addresses incur the incremental IP address charge in addition to the standard monthly
recurring charge for SDSL, iDSL, T-1, Fractional T-1, and ADSL SOHO Services. These IP address charges apply both at the time of the initial order and for any post-install IP change order (e.g., Customer wants to change from sixteen (16) IPs to thirty-two (32) IPs).
**For these configurations, Lightning Bolt uses two (2) static IP addresses to provide the Service and assigns one (1) static IP address to the router. A total of three (3) static IP addresses will be unavailable to the End User. NAT is not available for these configurations.


Other Non Recurring Charges for ADSL, SDSL, iDSL, T-1, and Fractional T-1 Services

Installation Charge. This charge includes provisioning of PVC, installation of RJ45 Wall Jack, installation and configuration of CPE (if it is purchased from or qualified by Lightning Bolt), testing of the End User Circuit and up to fifteen (15) minutes of installation or repair of inside wiring from RJ45 Wall Jack to NID or for tracing or toning across phone closets between the RJ45 Wall Jack and the NID. (The Service Setup Charge does not include CPE or inside wiring beyond the first fifteen (15) minutes.)

Field Technician Dispatch Charge. This charge is billed for the dispatch of a Field Technician to End User location to service non-Lightning Bolt service faults, Service upgrades and downgrades requiring the dispatch of a Field Technician. (The Field Technician Dispatch Charge does not include inside wire install or repair.)

Extra Time Charge. This charge is billed for each whole or partial fifteen (15) minute increment Field Technician is at the End User premises during a Service Setup (other than the time included as part of the Service Setup Charge) or Field Technician dispatch during which Field Technician provides services that include: installation or repair of inside wiring from RJ45 Wall Jack to NID or for tracing or toning across phone closets between the RJ45 Wall Jack and the NID; troubleshooting and/or repair of CPE, CPE configurations, or service setups. (These time charges are not included as part of the Service Setup or Field Technician Dispatch Charges.)

Cancellation Charge. This charge is billed for each End User Circuit order that is cancelled more than five (5) full business days after the acceptance of the order and prior to completion of service setup by Lightning Bolt.

End User Switch Charge. This charge is billed on each End User Circuit that Lightning Bolt switches from another qualifying DSL provider over to our network that does not involve the dispatch of a Covad Field Technician.

Missed Appointment Charge. This charge is billed when the Field Technician arrives at the End User premises and does not have full access to complete the installation of the End User Circuit, e.g., End User no-show, Lightning Bolt Field
Technicians lack of access to the network interface device, or End User request for installation rescheduling or cancellation with less than one (1) full business day advance notice.

ADSL Installation

Self-Installation includes:

• Instructions and packaging
• Ground shipping

* Field Service Technician Dispatch: If End User orders a Professional Installation after ordering the Self Installation, the End User will be charged a one-time fee for a Field Service Technician Dispatch per End User according to the table of Other Non Recurring Charge (see above chart). Lightning Bolt will charge the end user this fee each time Lightning Bolt dispatches a Field Service Technician to an End User in accordance with Lightning Bolt's policies for cancelled and missed appointments.

Professional Installation includes:

In addition to the equipment charges, Customer will pay the professional installation charges set forth in the table below:

Professional Install: A Lightning Bolt Installation including provisioning of End User Circuit and dispatch of a technician to End User premises where any of the following on-site services are performed: (Limit 2 hours total per Professional Install)
• NID splitter or in-line filter installation including cost of filters or splitter (Lightning Bolt's discretion on which is performed)
• Basic inspection of inside wiring for a maximum of thirty (30) min. Includes tracing or toning across phone closets between the NID and an existing RJ-11 Wall Jack where service will be installed.
• Installation of Lightning Bolt provided DSL Gateway Install Kit equipment
• Does not include installation or repair of inside wire except as required for NID splitter installation. End Users are responsible for inside wiring. Lightning Bolt does not offer general installation or repair of inside wire with Professional Install.
